Comparison of modified varicose vein stripping with gauge packing versus conventional stripping in varicose veins – A prospective study

Background: Varicose veins are commonly treated with great saphenous vein (GSV) stripping, traditionally followed by external compression. A modified technique using gauge packing may offer advantages in specific settings.
Aims and Objectives: This study aims to compare the outcomes of conventional GSV stripping with external compression versus modified stripping with gauge packing in the management of varicose veins.
Materials and Methods: This prospective study was conducted in the Department of General Surgery, Pt. B.D. Sharma Post Graduate Institute of Medical Sciences, Rohtak, from October 2022 to November 2023. Fifty patients with clinically and Duplex-confirmed varicose veins were randomly allocated into two groups. Group A (cases) underwent the Trendelenburg procedure with stripping of the thigh segment of the GSV and track packing using rolled gauge. Group B (controls) underwent the Trendelenburg procedure with conventional stripping and external compression. In both groups, perforator ligation and multiple superficial phlebectomies were performed, followed by external compression.
Results: Postoperatively, ecchymosis occurred in 8 patients (4 in each group). Wound infection was observed in 2 patients. One patient in Group A developed a hematoma due to inadequate compression. Overall complication rates were similar in both groups.
Conclusion: Both techniques are effective, with comparable complication rates. The modified technique is useful in settings with a limited workforce, as gauge packing provides immediate compression without continuous manual application. However, when adequate personnel are available, the conventional technique remains preferable for optimal compression control.
